In our office, we offer high quality care and payment options that work for all of our patients. Below you will find a list of insurance and payment options that we accept. Dental Insurance Plans We Work With:

We are often asked, “Do you accept my insurance”? This is complicated. Many insurance companies have multiple plans. While we may be in-network with certain plans, we might be out-of-network with other plans, and may not accept certain plans at all within the same insurance company. The best way to see if

we can receive payments from you or insurance company is to check with your HR department or the insurance company itself.

Another question often asked is, “Why don’t you accept all insurance plans”? In order to provide the best quality of care, certain plans within certain insurance companies would reduce our ability to give you the services you deserve. In order to keep our costs and your costs down, we are unable to accept every insurance plan We are more than happy to help you file your insurance, but that is not a guarantee your treatment will be covered.

Payment options:

• Cash or Check: Always accepted.
• Credit Card: Visa, MasterCard, Discover.
• CareCredit: Apply online (www.carecredit.com) or in our office.

Please note: Payment is due at the time of service.

As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
